Inflammaging Pathways
What is inflammaging?
Inflammaging is the chronic, low-grade inflammation that tends to increase with age. It is quieter than the inflammation of infection or injury, but over time it can contribute to disease, frailty, sarcopenia, and loss of physical reserve.
Why pathways matter
Inflammation is not one single chemical. It is a network of signals. The body uses inflammatory pathways to defend itself, repair tissue, and respond to stress. The problem comes when these pathways remain activated too often, for too long, or in the wrong setting.
Long on Meds focuses on three major inflammatory pathways. These pathways do not include every inflammatory signal in the human body, and they are not the only pathways that matter. But they are central pathways that strongly influence inflammatory activity, tissue injury, metabolic stress, vascular disease, and loss of physical reserve.

The three Long on Meds pathways
1. Cytokine Signaling / NF-κB
Cytokines are chemical messages used by immune cells and injured tissues. They help the body respond to danger, infection, stress, and tissue damage.
NF-κB is one of the major switches that turns inflammatory genes on. When this switch is activated repeatedly, the body may produce more inflammatory signals such as TNF-alpha, interleukin-1, interleukin-6, and other cytokines.
In plain language: this pathway is the body’s inflammatory broadcasting system. When it is overactive, the volume of inflammation stays too high.
2. NLRP3 Inflammasome
The NLRP3 inflammasome is part of the body’s danger-sensing system. It helps immune cells detect stress signals, damaged cells, joint crystals such as gout, kidney stones, metabolic stress, and other signs that tissue may be under threat.
When NLRP3 is activated, it can increase production of powerful inflammatory signals, especially interleukin-1 beta. This pathway has been linked to gout, vascular inflammation, metabolic disease, kidney disease, neuroinflammation, and age-related tissue injury.
In plain language: this pathway is one of the body’s internal alarm systems. It is useful when danger is real, but damaging when the alarm keeps ringing.
3. Oxidative Stress / ROS
Oxidative stress occurs when the body produces more reactive oxygen species, or ROS, than it can safely control. ROS are not always bad. The body uses them for normal signaling and defense. But excess ROS can damage proteins, fats, DNA, mitochondria, blood vessels, and muscle tissue.
Oxidative stress is closely connected to inflammation. Inflammation can increase oxidative stress, and oxidative stress can feed more inflammation. Together, they can create a cycle that contributes to aging tissues losing reserve.
In plain language: this pathway is the body’s rust-and-repair problem. Too much oxidative stress can wear down cells faster than they can recover.
Why these pathways are useful
These three pathways matter because they are not only central to inflammation; they are also pharmacologically approachable. Many established generic medications appear to influence one or more of these systems.
